New Patient Registration

You can register as a new patient at either site, Wyke or Lanehouse, our receptionists will give you all the forms required and any help/assistance you may need. Please bring your NHS number and check that you are living within our practice area, you will be asked to provide proof of your address.

Alternatively you can download the application forms, (links below, note that there are separate forms to be completed for adults and children under 16), complete and sign where appropriate and bring them to either surgery. 

All new patients are invited for a health check. This enables us to obtain details of your past medical problems and provide better care.

You will be given a named accountable GP, and wherever possible we ask that you see your own doctor. If you have a preference as to which doctor you are registered with, please express your preference to the receptionist when registering. The practice will endeavour to  comply with your wishes.

Out of Area Service-

We are now able to accept patients who live outside the practice boundary. However, strict criteria will apply on each individual basis and the patient will only be accepted if approved by your GP. To apply as an out of area patient you will need to visit either surgery and speak to one of our receptionists, or you can fill out this Form and send it to us. Please note that if you register under our Out Of Area Service you will not be entitled to home visits. To 

Change of Personal Details

Please notify the practice immediately if you change your name, address or telephone number so that we can keep your records up to date.

Contact Details

Please ensure that we have the correct contact details for you. Home phone number, mobile number and email address. We can now contact you by text and email.
